1. Voluntary Participation and Risk
Basketball and conditioning involve physical exertion and risks including falls, collisions, sprains, fractures, concussion, serious injury, illness, or other harm. The participant and parent or legal guardian voluntarily accept these risks.
2. Participant Health
The parent or legal guardian confirms that the participant is able to participate and will disclose relevant health conditions, allergies, medications, or limitations.
3. Release of Liability
To the fullest extent permitted by law, the participant and parent or legal guardian release and hold harmless West Valley Swish Academy, its owners, coaches, employees, volunteers, affiliates, sponsors, and facility partners from claims arising from ordinary participation risks.
4. Emergency Medical Authorization
If the parent or legal guardian cannot be reached, Academy representatives may obtain reasonable emergency medical assistance. The parent or legal guardian remains responsible for related expenses.
5. Concussion and Injury Awareness
A participant showing signs of concussion or significant injury may be removed and required to obtain medical clearance before returning.
6. Transportation
The parent or legal guardian is responsible for transportation unless a separate written arrangement exists.
7. Conduct and Safety
The participant must follow coach instructions, facility rules, safety requirements, and the Code of Conduct.
8. Media Choice
Media permission may be accepted or declined during registration. Written opt-out requests may be sent to swishacademy@outlook.com.
9. Acknowledgment
Electronic acceptance is intended to have the same effect as a written signature.
